Do you need father's permission for a pport for a child?

Hi my parents got divorced a long time ago and my mother has full custody. She is trying to get me American citizenship so I can get a American pport. I was born in the Czech Republic. We were wondering if we need my father's permission to obtain the pport. I am 16. I am in the US and I want to go to Canada.
